The Registration Process
You will be asked to complete an online, new patient paperwork packet prior to scheduling your first appointment. This paperwork includes a registration form, a medical history form and a consent for general dentistry form. Should you carry dental insurance, you will have the opportunity to provide us with the details so we can begin filing claims as of your first visit.

Once you’ve submitted your completed paperwork, we will call and schedule your appointment.
